We Provide Professional Passport Services
Passport Renewal
All Passport renewals require the completion of Form DS-82.
Note: We do not accept this form at any of the locations. It must be mailed to the address listed on the form.
You may renew your passport by mail if you can answer “yes” to the following questions:
- Do you already have a U.S. passport which is NOT damaged?
- Did you receive it within the past 15 years
- Were you at least 16 years of age when you received it?
- Do you still have the same name or legal proof of your name change? (You will need to submit this proof in an original format.)
- Do you already have a U.S. passport which is NOT damaged?
- Did you receive it within the past 15 years
- Were you at least 16 years of age when you received it?
- Do you still have the same name or legal proof of your name change? (You will need to submit this proof in an original format.)
You will need to include the following:
- Your most recent passport
- One passport photograph
- And, a $130.00 fee payable to the U.S. Department of State
If you need your passport urgently, you may request Expedited Service. Note: Your old passport will be returned with your new passport.

Child's Passport
- All children under age 16 must apply for a passport in person with two parents or guardians using Form DS-11.
- You cannot renew your child’s passport using Form DS-82.
- Passports for children under age 16 are only valid for 5 years.

Avoid the top mistakes on passport applications
We want to make sure you get your application right the first time. Make sure you:
- Complete all sections of your form including entering your correct Social Security number. Do not leave anything blank. If you’re applying for the first time or with your child under age 16, wait to sign the form until you are instructed to do so.
- Closely follow our passport photo requirements.
- Provide the correct evidence of U.S. citizenship.
Passport Card
We began issuing U.S. passport cards in 2008 to meet the needs of U.S. citizens who travel from Canada, Mexico, Bermuda, and Caribbean countries by land or sea. The passport card cannot be used for international air travel and is a cheaper alternative to the passport book. Steps to Apply for a Card
We use the same forms for both passport books and cards. The first step on our forms is to select if you want a passport book, passport card, or both documents.
Name Change
If your name has changed since your last passport was issued, you must include the following:
- A certified copy of the legal document specifying your name change (for example: a marriage certificate, a divorce decree, an adoption decree, or a court order).
- No photocopies will be accepted.
To ensure that you have included all requested documents required to process your application, please refer to Form DS-82 for a listing of these requirements.
Lost, Stolen or Damaged Passport
- If your passport is lost or stolen, you are advised to report the loss/theft to the local Indian Police Station and obtain a police report.
- Original documents will be returned to you after review of your application.
Report your passport lost or stolen immediately to protect yourself from identity theft. You are also advised to report the loss/theft to the local Indian Police Station and obtain a police report. Once you report a passport lost or stolen, it is invalid and cannot be used for international travel.
To renew a lost or stolen passport you are required to apply in person by attending a pre-arranged appointment. If you need an emergency replacement for immediate travel and there are no appointments available online send an email to the embassy or consulate near you.
In order to receive a replacement passports, please bring the following to your pre-arranged appointment:
- Completed (but not signed) Form DS-11 Passport renewal form (PDF version) : Application for a U.S. Passport. Checkout the steps to fill an online application form here.
- For lost/stolen/misplaced passports: completed and signed Form DS-64: Statement Regarding Lost or Stolen Passport.
- For damaged passports: signed written statement explaining how and when your passport was damaged.
- Report lost or stolen to the Indian police. Carry a copy of the police report to the appointment.
- One passport photo.
- Evidence of identity or citizenship.
- Applicants under age 16 are required to be present along with their parents. Please also bring the documents mentioned in the link here.

Lost Indian Visa
People who lose their passport with their valid Indian visa must use their new replacement passport to apply for an exit visa with the Government of India’s Foreigner’s Regional Registration Office (FRRO) Online Portal. This process takes approximately 7-10 business days. You cannot leave India without this visa. More information on obtaining an exit visa will be provided when you come to apply for a replacement passport. Exit visas are not issued on weekends or Indian holidays.
